How To Create a VHD?

I would like to create a new VHD of Windows XP Pro. Or Server 2003. Or
whatever.

I cannot for the life of me find documentation on how to do this. Plenty of
links telling me I need to create a .vhd that can host an operating system,
but no actual instructions.

Can someone point me in the right direction?

When you create a new VM, you get the option to also create a new VHD,
there is no need to do them separately.
